LIVE WELL OZAUKEE
Caring for someone else is the common thread that connects us all, and together we have the power to transform care and create stronger communities. At Aurora Health Care, we help people live well, because the only thing better than a healthy you, is a bunch of healthy yous. We do that in our hospitals, clinics and in partnerships with community organizations. We are committed to creating better care models and ultimately, helping people achieve the best quality of life possible - whatever the diagnosis.
By supporting Live Well Ozaukee, you will change the culture of care and start the conversation to determine what matters most to patients and their families facing medical decisions, how to have goals of care conversations ahead of a medical crisis, and how a palliative care team work’s towards life goals in a patient centered and family engaged care plan.
We invite you to join Live Well Ozaukee and enjoy a “farm to fork” gourmet dinner, interactive stations, entertainment and silent auction packages, while raising funds to support Aurora Medical Center Grafton’s Well Community palliative care programs and services. With unique day-of event recognition and visibility for 275+ guests, your support sends a strong message that together, we will transform the culture of care in Ozaukee County.
